Optimal care of kidney transplantation recipients requires a coordinated treatment and management approach to guide transitions of care from the inpatient to outpatient care setting; emergency department visits can represent fragmentation in this coordination of care. A more definitive understanding of the trends and causes of ED utilization among a national population of kidney recipients is necessary to identify potentially modifiable factors and to identify opportunities to improve quality of care and lower resource utilization. This activity will develop transplant providers’ understanding of the emergency department utilization, hospital admission, and outpatient nephrology visits, and to determine the sociodemographic and clinical characteristics associated with emergency department services following kidney transplantation.
Learner Objectives
1. Assess the epidemiologic burden of emergency department utilization among kidney transplant
recipients
2. Evaluate patient, clinical, and transplant-specific factors associated with higher emergency
department utilization
3. Counsel kidney transplant recipients on common post-transplant complications associated with
emergency department utilization and indications for escalation of acute care
4. Develop and test interventions to optimize emergency department management and care
integration for kidney transplant recipients
